Assisted Living Program
The Assisted Living Program, previously known as Supportive Housing, serve the needs of adults who have a physical disability and are able to independently direct and manage their activities of daily living and personal care. Each individual resides in their own home and requires the availability of personal support and homemaking services on a 24-hour basis.

Personal Support Services
Personal Support Services assist individuals with activities of daily living such as personal hygiene, dressing, mobility, and nutrition to help them remain safely in their homes or community settings. These services are delivered by trained PHARA team members and are designed to support health, independence, and overall quality of life.Homemaking Services
Homemaking Services provide assistance with routine household activities such as cleaning, laundry, meal preparation, and shopping to help individuals maintain a safe and healthy living environment. These services support individuals who may be unable to manage these tasks independently, allowing them to continue living in their own homes.Security Checks and Reassurance Services
Security Checks and Reassurance Services provide involve regular contact, either in person or by phone, to confirm an individual’s safety and well-being. These services are designed for individuals who may be at risk due to age, illness, or isolation, offering peace of mind through consistent monitoring. The service provides both clients and family with reassurance that if concerns arise, PHARA staff may take appropriate action, including contacting emergency services or notifying caregivers.
PHARA Community Services operates the Assisted Living Program in both community and campus-based models in North Bay and surrounding communities.
The community-based program requires a client to live in their home, require scheduled and unscheduled services 24 hours per day, 7 days per week, and live within a designated geographical area. Access to community-based Assisted Living is through Ontario Health at Home.
The campus-based program is available in a PHARA-operated building that has dedicated Assisted Living units, such as 122 Massey Drive, 200 Oakwood Avenue, and 280 Oakwood Avenue. Access to campus-based Assisted Living is by referral from a health care agency or self-referral.
