Post Stroke Transitional Care Program

The Post Stroke Transitional Care Program supports recovery of stroke clients and families by eliminating barriers and providing education. In addition, the PHARA Community Services team may also help in facilitating connections to services such as:

  • Transportation

  • In-home nursing

  • Personal care

  • Adaptive equipment

  • Home modifications

  • Community engagement

  • Physical, occupational, speech, and mental health therapies

The program also directly offers rehabilitative services such as individual and group exercise, support groups, assessment, cognitive rehabilitation, and life- skills retraining.

The Post Stoke Transitional Care Program implements a number of Stoke Best Practice Guidelines, notably supporting stroke clients through transitions and facilitating community re-integration. Clients that utilize the program’s navigation services require fewer visits to the emergency department, are less likely to be admitted to the hospital, and experience improvements to their overall sense of well-being.

As part of the Post Stoke Transitional Care Program, PHARA Community Services provides:

  • TIME™ – Together in Movement and Exercise
    This 12-week group exercise program supports individuals in maintaining an active, independent lifestyle by improving mobility, strength, balance, and social engagement. (Developed by the Toronto Rehabilitation Institute and Toronto Parks, Forestry and Recreation.)

  • FAME® – Fitness and Mobility Exercise
    A group exercise program designed for individuals living with physical disabilities or post stroke conditions, targeting balance, strength, mobility, cardiovascular fitness, and mental health. The evidence-based FAME® program improves physical and cognitive function while reducing the risk of falls, fractures, and heart disease. (Developed by Janice Eng, PhD PT/OT in Vancouver, BC.)

Access to Post Stoke Transitional Care Program is by referral from the North Bay Regional Health Centre or a Health Services Provider.
