REHABILITATION SUPPORT WORKER
(Post Stroke Transitional Care Program)

Permanent Full-Time; $23.87 - $25.02/hour

KEY RESPONSIBILITIES

The RSW assists/teaches clients in their rehabilitative activities including life skills so that maximum independence and identified goals are achieved. The RSW will lead individual physical and cognitive rehabilitation sessions as well as facilitate exercise groups under the authorization of an OT/PT authorized to work in Ontario.

QUALIFICATIONS

  • College diploma in OTA/PTA required

  • Experience with stroke rehabilitation

  • Able to work in a professional manner independently as well as with direction required

  • Group facilitation skills

  • Excellent reporting skills

  • Experience and knowledge in working as a member of an interdisciplinary team preferred

  • Experience working with individuals who are frail, elderly, and have physical disabilities required
