Services


ATTENDANT CARE SUPPORTIVE HOUSING

Tenants manage their own lifestyle, and are encouraged to participate and become active members of our community. Attendants are available 24 hours a day to assist with personal care needs, homemaking services and other activities associated with daily living needs. The program is offered in designated units within buildings operated by PHARA known as Westwinds Apartments, Westwinds Heights, and Westwinds Village in North Bay.  The program is also offered by PHARA at 135 Dawson St., Thessalon, Ontario.

ATTENDANT OUTREACH

The program provides personal support services to adults in the North Bay, and surrounding communities.  Attendants provide assistance associated with daily living needs.  Services are available at pre-scheduled periods to assist with personal care needs, homemaking services and other activities associated with daily living needs.  Services are available from 6 am to midnight.  

Attendant Care Outreach also includes the Assisted Living for High Risk Seniors program.  Note: Access to the Assisted Living for High Risk Seniors program which includes access to 24 hour support is by Home and Community Care referral only.

TRANSITION TO HOME PROGRAM

Our Transition to Home Program assists clients on a short term basis (90 days) in transitioning from the hospital to home, or hospital to long term care or a retirement facility. The clients are accommodated within a 2 bedroom fully furnished unit with shared common and living areas. Attendants are on site 24/7, 365 days a year to provide assistance with daily living.

Note: A daily rate is charged for this program.  This includes rent, groceries, cable, phone, laundry and cleaning supplies.  Fees may apply. 

This program is in collaboration with, and clients are referred by the North Bay Regional Health Center.

ENHANCED CONGREGATE CARE

PHARA has two designated units for the Enhanced Congregate Care program for individuals deemed ‘ALC’ for use on a short term basis (90 days). In this shared living area program each unit has two bedrooms, with common areas and a bathroom.  PHARA provides support 24/7 365.  

Note: A daily rate is charged for this program.  This includes rent, groceries, cable, phone, laundry and cleaning supplies.  Fees may apply.

This program is in collaboration with, and clients are referred by the North Bay Regional Health Center.

LOAN OUT CUPBOARD

We provide a free of charge service to individuals in the community who are in need of assistive devices. Members of the community are able to loan out an assisted device from PHARA on a 6 month period. Our Loan Out Cupboard is stocked with assisted devices through donations from people in our communities. We have access to a variety or equipment including walkers, wheelchairs, canes, and much more.  This program operates based on donations from people like you. Contact PHARA for further information.

MEALS ON WHEELS

PHARA operates the Meals on Wheels program in Thessalon, Thessalon First Nation, Iron Bridge, and Bruce Mines. Meals are available at cost one to three times per week on Monday, Wednesday and Friday. This service is available for anyone with difficulty preparing meals.

POST STROKE REHABILITATION

The goals of the program are to maximize stroke recovery and provide multiple avenues for successful community reintegration. Offering individuals a comprehensive continuum of services, including education, socialization, recreation, assessment and therapy. As well as connecting individuals to needed community health and social services.

In partnership with North Bay Regional Health Center.

MONTHLY STROKE SUPPORT GROUP

A social and educational support group for persons who have suffered a stroke, their friends and family. This group meets one morning a month to participate in group activities, and share experiences while offering support to one another. Fees may apply.

STROKE COMMUNITY NAVIGATION SERVICE

Our Stroke Community Navigator will facilitate connections to all kinds of stroke based services in the community and community connections for medical appointment and recreational pursuits. We provide help in finding transportation solutions, in-home nursing, personal care and housekeeping, physical activities programs, or other services offered by various agencies in the community.   

LIVING WITH STROKE

Living with Stroke is an 8 week support program for anyone who has had a stroke and their caregiver. In partnership with the Heart & Stroke Foundation of Canada.

TIME

The TIME program is an exercise program that comprises of 2 sessions a week for 12 weeks, exercise programs are designed by physiotherapists, exercise circuit begins with seated warm up (5 min), there 9 stations in a circuit (40 min), and a cool down at the end of the circuit (10 min). Fees may apply.

FAME

This group exercise program designed in Vancouver, Canada by Janice Eng PhD PT/OT addresses the multiple impairments that arise from the multiple chronic health conditions of stroke including balance, muscle strength, bone health, mobility, cardiovascular fitness and depression. The FAME program has been proven to improve the physical and cognitive abilities of people living with a stroke and reduce the risk for secondary complications such as falls, fractures and heart disease. Fees may apply.

COGNITIVE REMEDIATION PROGRAM FOR STROKE

Learn strategies for improving your memory, concentration, organizational abilities, communication, reading, writing, and other life skills. Fees may apply.
